Position Summary
The GUIDE Clinical Specialist is a critical clinical and operational leader supporting Bloom’s participation in the CMS GUIDE Model (Guiding an Improved Dementia Experience). This role leads patient outreach, facilitates enrollment processes, develops comprehensive care plans, and serves as the internal clinical subject matter expert for dementia-related services across the organization.

The GUIDE Clinical Specialist operates at the intersection of patient engagement, care coordination, compliance, and external partnership management—ensuring regulatory requirements are met while delivering a seamless experience for patients and families.

Key Responsibilities:

1. Lead Initial Patient Outreach & Enrollment
  • Conduct outreach to:
    • New-to-Bloom patients
    • Existing Bloom patients with a new dementia diagnosis
  • Introduce eligible patients and families to the GUIDE Program and assess interest.
  • Complete comprehensive care plans for eligible and consenting patients.
  • Collaborate cross-functionally to schedule GUIDE-specific provider visits.
  • Maintain accurate documentation and tracking of:
    • Outreach attempts
    • Enrollment declines
    • Completed care plans
    • GUIDE provider visit completion
2. Manage CMS Beneficiary Alignment & Notifications
  • Conduct outreach to new GUIDE beneficiaries to:
      • Notify them of enrollment
      • Complete or review comprehensive care plans
      • Provide immediate education and resource guidance
      • Send appropriate referrals
      • Facilitate seamless hand-off to the assigned Nurse Care Manager
  • Conduct outreach to beneficiaries rejected from alignment:
    • Clearly communicate reason for rejection
    • Ensure documentation meets compliance standards
3. Serve as Bloom’s GUIDE Clinical Subject Matter Expert
  • Provide ongoing clinical guidance and consultation to Nurse Care Managers (NCMs):
    • Resource navigation for complex dementia cases
    • Support with caregiver engagement strategies
    • Assistance in meeting monthly communication and documentation requirements
  • Partner with Intake and Reception teams:
    • Introduce GUIDE services to prospective patients
    • Ensure accurate representation of eligibility criteria and program scope
4. Facilitate Respite and Education External Partnerships
  • Serve as the clinical liaison for respite care partners.
  • Coordinate with education program partners to:
    • Track referrals
    • Monitor engagement
    • Strengthen collaboration
  • Lead quarterly partnership check-ins to ensure quality, performance alignment, and continuous improvement.
Qualifications
  • Active RN or other relevant clinical licensure (state-specific, as applicable).
  • Experience in dementia care, geriatrics, home-based care, or care management strongly preferred.
  • Working knowledge of CMS models and value-based care structures preferred.
  • Strong documentation, regulatory compliance, and workflow management skills.
  • Demonstrated ability to collaborate across interdisciplinary teams.
  • High emotional intelligence with strong caregiver communication capabilities.

Compensation
Falls between $95,000-$105,000 annually, depending on experience, with opportunity for up to 10% performance-based bonuses and full benefits.
