Hospice IDT Coordinator Responsibilities:
  • Scheduling Duties:
  • Create and manage schedules for RN case managers, CNAs, social workers, and other care team members to ensure all patient needs are met.
  • Match patient needs with the appropriate healthcare professional, ensuring that visits are timely and that staff assignments align with patient care plans.
  • Appointment Management: Schedule patient visits, treatments, and any necessary follow-up appointments with healthcare providers.
  • Communication: Remain the central point of contact between staff, patients, and families. Ensure that staff are informed of any changes to schedules, patient conditions, or specific care needs.
  • Own team performance measures that translate to exceptional patient outcomes, including care team productivity, patient satisfaction, and timely answering of calls
  • Handling Emergencies by quickly finding a replacement care team member to ensure continuity of care.
  • Handle some administrative tasks, such as answering phones, maintaining records, and managing communication with various departments.
  • IDT Management:
    • Create and manage IDT groups.
    • Review IDT notes and ensure timely completion of required signatures.
    • Send out IDT note completion summaries, starting the Thursday before IDT and providing updates on Monday and Tuesday.
    • Incorporate Pharmacy and DME data into the IDT schedule.
    • Scribe all IDT meetings, ensuring accurate documentation.
    • Distribute Plan of Care (POC) documents to the community every Thursday.
    • Provide follow-up summaries to individual IDT members.

  • Excellent phone/customer service skills
The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $25.00 - $27.00 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
