Position Summary
The Hospice Intake Coordinator Team Lead plays a pivotal role in ensuring a seamless referral-to-admission process for Bloom’s hospice program. This position is responsible for overseeing daily intake operations, supporting the intake coordinators, and collaborating closely with clinical, operations, and sales teams to ensure timely and compassionate onboarding of patients and families. The Team Lead provides leadership, mentorship, and process oversight to ensure the intake team meets organizational standards for quality, efficiency, and compliance.
Key Responsibilities
- Oversee day-to-day hospice intake operations, ensuring timely processing of referrals, eligibility verification, and coordination with interdisciplinary teams.
- Serve as the primary point of contact and escalation resource for intake coordinators, assisting with complex referrals or process questions.
- Collaborate with hospice leadership to identify workflow gaps, optimize processes, and improve the overall intake experience.
- Review and monitor referral trends, conversion rates, and team metrics to ensure timely follow-up and goal achievement.
- Train and mentor new intake coordinators, ensuring consistency in documentation, communication, and service standards.
- Communicate effectively with providers, referral partners, and internal departments to ensure smooth transitions of care.
- Maintain accurate documentation within the EMR and ensure compliance with all regulatory and organizational policies.
- Partner with the Hospice Clinical Director and Intake Manager to standardize best practices across markets and support onboarding initiatives for new territories.
- Provide coverage for intake team members during absences or high-volume periods as needed.
- Support the development of a positive, team-oriented culture that aligns with Bloom’s mission and core values.
Qualifications
Education & Experience:
- Current Bloom Employee
- Minimum 1–2 years of hospice intake, admissions, or healthcare coordination experience required.
- Prior leadership or team lead experience preferred.
- Working knowledge of Medicare hospice eligibility requirements and referral processes strongly preferred.
Skills
- Excellent organizational and communication skills with strong attention to detail.
- Ability to lead and motivate a team while managing multiple priorities in a fast-paced environment.
- Strong understanding of EMR systems, referral management platforms, and Microsoft Office Suite.
