Job Summary: 

Nurse care managers are responsible for managing complex care situations, ensuring seamless communication among all stakeholders, and providing exceptional service to patients, families, and healthcare teams. This role requires a high degree of clinical expertise, strong organizational skills, and the ability to balance multiple tasks while maintaining a patient-centered approach. The nurse care manager is expected to handle complex cases with critical thinking and deliver outcomes that improve patient care and satisfaction.  

• Understands and creates patient care plans. 

• Updates patient specialty and comprehensive care plans to meet the needs of the patients and providers.  

• Coordinates care with community partners and teammates. 

• Coordinates and tracks transitions of care.  

• Provides exceptional professional customer service to patients, families and community partners.  

• Collaborates and coordinates with the Interdisciplinary Care Team (IDC) to provide excellent patient care.  

• Supports and implements quality improvement programs.   

Nurse Care Manager Knowledge/Skills/Abilities: 

• Strong computer skills and performance tracking 

• Ability to multi-task at a very high level and manage time efficiently 

• Strong problem-solving abilities 

• Excellent phone/customer service skills 

• Strong Clinical Background 

• Flexibility and adaptability to changes 

• Ability and willingness to expand clinical and general healthcare knowledge 

• Strong critical thinking skills 

• Self-motivated  

• Experience in healthcare coordination, case management or similar role. 

• Experience in out-patient/ambulatory/home health environment 

Required: 

• Active professional license (RN, LPN) 

• Minimum 3 years of clinical experience.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between LPN $28-32 hourly / RN $39-44 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
